For the very first time, IMI Pakistan Karachi Chapter organized a Webinar with one of the renowned Urological Surgeon from Ireland, UK. Dr. Qasim Jaffrey. He began with the historical aspects of stone management. Dr. Jaffrey discussed important medical terms to create understandings of the participants. He highlighted that the stone does not cause pain but the obstruction leads to severe pain in ureter called Ureteric Colic. Discussing the investigation, he said CT Scan is the standard for confirming renal stone. He showed X-Rays of renal stone patients which helped in identification of renal stone. He also emphasized upon the analysis of stone once it is removed from the body. Later on, he explained different types of stones, their composition, radiological fixings and morphology. He also distinguished the stones on the basis of their shapes. He covered the signs and symptoms of nephrolithiasis which commonly includes back pain, if stone obstructs in ureter. The pain typically occurs at night or early morning. He then moved towards treatment and according to him, there are multiple treatment options depending on the size of stone. First option is conservative management by analgesics and increased uptake of water, mostly done in stones of size <4 mm. Stones >4 mm to <1.5 cm can be treated with Extracorporeal Shock Wave Lithotripsy (ESWL) whereas the stone which is greater than >1.5 cm is treated with Percutaneous Nephrolithotomy.
